If you need to bring prescription medications, please bring them to the nurse's office in the original container with the prescription label and sign a medication form. Medication cannot be administed without a signed permission form. Med - RX (2).docx

If your child has Asthma/Anaphylaxis Allergies please come by for a health plan form to be signed by your child's doctor. This needs to be signed in order for your child to have permission to carry an inhaler with them. Ashtma Anaphy Action Plan (one page revised).xlsx

Please remember if your child has a food allergy to bring a doctor note yearly stating the allergy in order for it to be flagged at school.

If you have any updates on immunizations please turn those into the nurse’s office.
